It wouldn't need to be a site-to-site VPN - a remote access VPN from the client PCs (running the initiators) to the home network would also work. Either way, a VPN also provides better security than port forwarding. FWIW, I agree that iSCSI won't perform very well over most internet (WAN) connections.
